Position Summary:
Provides supportive psychosocial services to patients and families. Provides pre-death bereavement services, addressing such issues as family grief and loss, behavioral needs, and risk assessments for potential complicated grief. Provides services utilizing principles of palliative care and the hospice philosophy. Provides direct patient/family counseling and referrals to appropriate community resources as needed. Assists the patient and family in developing and enhancing optimal coping strategies. Supports the hospice philosophy and team approach to provide, with other members of the team, holistic care to the patient and family.

Duties & Responsibilities: 
  • Assesses psychological, social, grief, and environmental needs of the patients and families. Initial contact shall be made within 72 hours of admission. All care is provided in the patient's residence.
  • Provides social work services, including but not limited to: psychosocial assessment, counseling, consultation, education, resource and referral services, advocacy, and discharge planning, based on the social work assessment and plan of care, with the goal to empower the patient and family through education and support.
  • Participates in developing the interdisciplinary care plan both as a consultant and as a provider of direct services. Participates in the weekly interdisciplinary team conferences.
  • Responsible for the initial post death bereavement assessment and coordinates after-death bereavement plan of care with Bereavement Counselor.
  • Organizes, conducts, and/or participates in educational forums, seminars and/or in-service training’s concerning psychosocial and/or bereavement issues for hospice staff, volunteers, health care professionals and the public at large. 
  • Actively participates in Interdisciplinary Team meetings, contributing to the development and ongoing updating of the plan of care, including the bereavement plan for the family.
  • Documents all social work interactions in accordance with Hawaii Care Choices's regulations; maintains documentation of time sheets, mileage, and other records as required.
  • Provides bereavement consultation and support in response to community needs as assigned.
  • Upholds and maintains the organization's cost containment efforts, without jeopardizing the quality of hospice care.
  • Observes and promotes sound safety practices; follow all policies and procedures of Hawaii Care Choices. 
  • Provides education and training to the patients, care givers, volunteers, co-workers and the community at large as deemed necessary and as requested; participates in meetings, committee program development and special projects. 

Skills & Knowledge Requirements: 

  • Ability to exercise independent sound judgment in planning and providing quality psychosocial care.
  • Possession of critical assessment and thinking skills.
  • Ability to provide unsupervised psychosocial services and support in private residences.
  • Self-motivation and ability to work independently as well as in cooperation with the interdisciplinary team.
  • Interest and desire to give hospice care with a special sensitivity to the needs of the dying.
  • Maintains adequate education and experience level to meet the needs of the patients, care givers, and regulations established by State and Federal laws.
  • Possesses and maintains a valid Hawai’i State Social Worker's license.

Education & Experience Requirements: 

  • Graduate of a school accredited by the Council of Social Work Education, with a minimum of a Masters degree in social work.
  • Hospice experience and LCSW provider preferred.
  • Frequent use of private vehicle, computer, telephone, pager, and personal cellular phone (cell phone use prohibited while driving) and standard office equipment. 

Hours: 

  • Exempt position requires flexibility and may be required to work after-hours, on weekends, and holidays as needed. 

Pay: $69,000 - $70,000/ annually
This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
